Step 3. Click on CONTACT US.
Step 4. Meanwhile, when you click on Contact Us, a pop-up form is going to appear.
Step 5. Now on the pop-up form, click on “If you are a Member of the e-Rewards Opinion Panel, please CLICK HERE”.
Step 6. Write your FIRST NAME, LAST NAME, EMAIL ADDRESS.
Step 7. While on the Subject section, select MEMBERSHIP CANCELLATION REQUEST.
Step 8. And on the Question / Issue section, write your reason to delete your account.
Step 9. Finally, click on SUBMIT.
